Medicines In School
Administration of Medicine Form
Please complete this form if you wish your child to be given medication (PRESCRIBED BY THE CHILD'S DOCTOR). The medicine will be given at 12.00noon for KS1 and 12.15 pm for KS2. The form must be handed in to the school office with the medicine.
Leave of Absence During Term Time
If you wish to notify us of any leave of absence during term time, please complete the MS Forms link below, this includes holiday, dentist, doctors any request for leave at all:
